ABSTRACT

Objective:To investigate the feasibility of individualized primary clinical target volume (CTV) delineation in intensity-modulated radiotherapy for nasopharyngeal carcinoma (NPC).Methods:Clinical data of 87 consecutive patients newly diagnosed with lateralized NPC in Jiangsu Cancer Hospital between October 2016 and February 2018 were retrospectively analyzed. Lateralized NPC is defined as tumor invasion not exceeding the contralateral wall. According to the tumor spread, the primary CTV was optimized as follows: CTV2 only covered the medial part of the contralateral pterygopalatine fossa, whereas the contralateral foramen oval was not included; on the level of parapharyngeal space, the contralateral side of CTV only covered the posterior lateral lymph nodes, whereas the contralateral internal jugular vein was not regularly covered. Failure patterns and 5-year survival [local control rate (LCR), progression-free survival (PFS) and overall survival (OS)] were evaluated by Kaplan-Meier method. Paired t-test and rank-sum test were used to analyze the dose variation in the optimized region and adverse reactions. Results:The median follow-up time was 59.5 months. The 5-year LCR, PFS, and OS were 98.9%, 86.5% and 92.1%, respectively. There was no local recurrence in the optimized area of CTV. Dosimetric comparison results showed that the doses of parotid gland, temporal lobe, cochlea and middle ear on the contralateral side were reduced by 13.45%, 9.14%, 38.83%, and 29.36%, respectively. Four cases (4.6%) developed grade 3 hearing loss, all on the ipsilateral side. The optimized scheme significantly alleviated the hearing loss on the contralateral side compared to that on the ipsilateral side ( P<0.001). Other grade 3 late adverse reactions included cranial nerve injury, subcutaneous fibrosis in the neck and visual impairment, with 1 case each. Conclusion:Individualized primary CTV for lateralized NPC is feasible and safe, with obvious dosimetric advantages and reduced adverse reaction rate, which is worthy of clinical promotion.

ABSTRACT

Primary lung cancer, as one of the most commonly diagnosed malignancies and the top leading cause of cancer death worldwide, has seriously endangered human life. In China, due to regional differences, although the impact of lung cancer on different regions is different, it still has the highest mortality among malignant tumors. Western medicine often has certain limitations in the treatment of lung cancer. At present, the combination of western medicine and traditional Chinese medicine (TCM) or the direct application of TCM has become an inevitable trend in the treatment of lung cancer. TCM treatment is guided by the holistic view, which includes the view of integration of the five internal organs and the integration of the body and spirit. Therefore, the treatment of lung cancer in TCM emphasizes that although the disease is located in one organ, the whole body should be treated, which corresponds to the theory of the view of integration of the five internal organs. In addition to the lung in the treatment of lung cancer, the spleen and kidney should also be treated. The lung, spleen, and kidney correspond to metal, earth, and water, respectively on the five elements. According to the generation relationship of the five elements, since earth generates metal and metal generates water, earth is the mother of metal and water is the child of metal. Therefore, benefiting fire (kidney) can replenish earth. Tonifying the mother and son of the lung can treat lung diseases, reflecting mutual generation and transformation and ceaseless generation and transformation. Long-term consumption of Qi by lung cancer can affect mother-organ. In the context of disorder of child-organ affecting mother-organ, mother-organ should be tonified in the face of deficiency. The treatment principle of banking up earth to generate metal should be followed to reinforce healthy Qi and treat the root. Effulgent earth can promote the generation of metal and the syndrome of lung deficiency can be resolve. The lung and kidney deficiency is an important factor in the incidence of lung cancer. Yin deficiency for a long time can hinder the generation of water by metal, resulting in failure of water in moistening metal, so both mother organ and child organ should be tonified. The treatment principle of mutual generation between metal and water should be employed to moisten the kidney and lung, thereby facilitating the vigorous ventilation and descending of the lung. The spleen is the mother organ of the lung, and the kidney is the child organ of the lung. Long-term lung cancer causes the deficiency of the mother and child organs, so the treatment principle of replenishing fire to nourish earth should be adopted to benefit the kidney and tonify the spleen. Therefore, innate basis and acquired foundation are both filled, and the lung can be regulated properly. When the lung, spleen, and kidney are harmonized, the generation and transformation of Qi and blood are active, and the Yin and Yang of Qi and blood are balanced, so the self-rehabilitation ability of the body is ensured, and the treatment of lung cancer will achieve a good curative effect. According to the holistic view of TCM, the whole body should be regulated to treat lung cancer. The lung, spleen, and kidney are all treated in the treatment of lung cancer. With the lung, spleen, and kidney as the central axis, the treatment principles of banking up earth to generate metal, replenishing fire to nourish earth, and mutual generation between metal and water should be followed in the treatment of lung cancer, and a good therapeutic effect can be achieved.

ABSTRACT

ObjectiveTo observe the effect of bilateral sequential repetitive transcranial magnetic stimulation on the motor function of upper limbs in stroke patients. MethodsFrom December, 2020 to December, 2022, 62 stroke inpatients in the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine were included. They were randomly divided into control group (n = 31) and observation group (n = 31). Both groups accepted conventional medicine and rehabilitation, as well as electroacupuncture antagonistic muscle therapy. Before electroacupuncture, the observation group acceped low-frequency repetitive transcranial magnetic stimulation at primary motor cortex (M1) on the healthy side, followed by intermittent theta burst stimulation at M1 on the affected side, for four weeks. They were assessed with Fugl-Meyer Assessment-Upper Extremities (FMA-UE), modified Barthel Index (MBI) and modified Ashworth Scale (MAS), and motor evoked potential (MEP) latency was compared before and after treatment. ResultsOne patient dropped down in the observation group, and no adverse event happened. After treatment, the scores of FMA-UE and MBI significantly increased (|t| > 9.953, P < 0.001), and the score of MAS and the latency of MEP significantly decreased (|t| > 5.043, P < 0.001) in both groups; while all of them were better in the observation group than in the control group (|t| > 2.237, P < 0.05). ConclusionBilateral sequential repetitive transcranial magnetic stimulation can effectively promote the recovery of upper limb motor function in stroke patients.

ABSTRACT

Objective@#To study the effects of atmospheric fine particulate pollution on the lung function of primary school students before and after heating during the winter in Zhengzhou.@*Methods@#In Zhengzhou, two areas with low and high level of PM 2.5 pollution(A and B), were selected as monitoring points from 2016 to 2018. Each monitoring station selected one elementary school within around 1 km and used a cluster random sampling method to extract students from grades 3 to 5 as the research subjects. Lung function tests were conducted before and after heating in winter, and mass concentration of PM 2.5 were recorded daily and compared to those recorded one month prior.@*Results@#The average daily mass concentration of PM 2.5 were 74 μg/m 3 and 92 μg/m 3 in the light pollution monitoring points A and the heavy pollution monitoring points B, which exceeded the standard for 97 and 126 days, respectively. The FVC and FEV1.0 indexes for the first test of male students before heating were higher than those for the second test from 2016-2018 (P<0.05). Except in 2016,the FVC and FEV1.0 indexes for the first test of female students were also higher than those for the second test (P<0.05). After stratified by sex, multivariate linear regression showed that PM 2.5 was associated with lung function as measured by the FEV1.0, PEF, FEF25 and FEF75 values of the students were negatively correlated (B=-0.13--0.07,-0.13--0.08,P<0.05).@*Conclusion@#Fine particulate air pollution before and after heating during the winter in Zhengzhou had different effects on the lung function of primary school students. Therefore, it is necessary to strengthen the respiratory health protection of primary school students in winter to protect their health.

ABSTRACT

Objective@#To grasp the present situation of occupational hazards of coal dust in our country, understand our country coal dust workers' occupational health risks, provide information based on evidence and analysis for the government and organize to effectively deal with the current status of high coal workers pneumoconiosis incidence in China, and protect coal dust workers' occupational health. @*Methods@#The research object is the " mining-transportation-use" of coal industrial chain, referring to 33 units. Use field investigation to obtain the coal dust exposure, dust prevention measures and the occupational health data of study object. Use quantitative evaluation method of International council on mining and metals occupational health risk assessment model (ICMM method) and occupational hazard risk index method (index method) , with coal workers pneumoconiosis as health outcomes, to evaluate the coal dust occupational health risks of coal industrial chain. @*Results@#The free silica content of partial coal dust in China is more than 10%, and even to 19.5%. coal dust concentration in workplaces, such as excavating system of dust coal mining (total dust: 22.1~46.5 mg/m3, respiratory dust: 8.4~17.7 mg/m3) , dumper (total dust: 25.2 mg/m3, respiratory dust: 6.9 mg/m3) , transfer tower (total dust: 35.4 mg/m3) of coal transportation and belt coal plough device of coal use (total dust: 36.3 mg/m3, respiratory dust: 14.0 mg/m3) , are much higher than those in other workplaces, and coal dust concentration of workers in these places (2.6~9.3 mg/m3) are much higher than those in other places, which are statistically significant. ICMM method evaluation results show that the risk value of excavating system is between 504~1 089, and the risk value of comprehensive mining system is between 347~2 040, which are far statistically significant higher than that of other systems. Index method evaluation results (excavating system risk value between 3.1~9.7, fully mechanized system risk value between 3.7~9.3) , are basically identical with ICMM method (correlation coefficient r=0.857, P<0.01) . The new cases of coal worker pneumoconiosis are distributed in three post of coal mining, excavating and coal mine mixing. @*Conclusion@#Coal-dust hazards are widely distributed in the coal "mining, transportation, and use" industrial chain, which of the underground coal mine is as serious as intolerable, meanwhile the risk of other industrial chain is basically can tolerable. The high coal dust concentration and the high risk of coal dust occupational hazard are concentrated in the excavating system and the comprehensive mining system. It is recommended to start the study on risk assessment and risk response of coal dust health hazard at the national level, and the occupational exposure limit of coal dust should be established according to the content of free silica.

ABSTRACT

The American Association for the Study of Liver Diseases (AASLD) updated and published the Practice Guidance for the Diagnosis and Management of Nonalcoholic Fatty Liver Disease (NAFLD) in July 2017, which provides recommendations for the accurate diagnosis, treatment, and effective prevention of NAFLD. Related metabolic diseases should be considered during the initial evaluation of patients suspected of NAFLD. Noninvasive diagnostic techniques including transient elastography, magnetic resonance elastography, and serum biochemical models should be used to evaluate the development and progression of liver fibrosis in patients with NAFLD. Clinical liver pathology report should clearly differentiate between nonalcoholic fatty liver (NAFL), NAFL with inflammation, and nonalcoholic steatohepatitis (NASH) and identify the presence or absence of liver fibrosis and its degree. Early medication for NAFLD can only be used in patients with pathologically confirmed NASH and liver fibrosis, and it is not recommended to use pioglitazone and vitamin E as the first-line drugs for patients with NASH which has not been proven by biopsy or non-diabetic NASH patients. Foregut bariatric surgery can be considered for obese patients with NAFLD/NASH who meet related indications. It is emphasized that the risk factors for cardiovascular disease should be eliminated for NAFLD patients. Statins can be used for the treatment of dyslipidemia in patients with NAFLD/NASH, but they cannot be used in patients with decompensated liver cirrhosis. Routine screening or hepatocellular carcinoma surveillance is not recommended for NASH patients without liver cirrhosis. Cardiovascular disease should be taken seriously during liver transplantation evaluation. There is still no adequate clinical evidence for the treatment of NAFLD in children and adolescents, and intensive lifestyle intervention is recommended as the first-line therapy for such patients.

ABSTRACT

Objective@#To analyze the genome characteristics of an avian influenza A (H9N2) virus isolated from an 11-month-old infant, and to look for possible sources of infection.@*Methods@#Throat swabs were collected from an infant with influenza-like illness in influenza sentinel surveillance hospitals and isolated for influenza viruses using cells. The isolates were identified for influenza virus types and subtypes by the method of hemagglutination assay, hemagglutination inhibition assay and fluorescence PCR. Whole genome sequencing of the isolated virus was carried out. The genome nucleic acid sequences and the deduced amino acid sequences were analyzed by comparing the phylogenetic trees which were constructed by bioinformatics software.@*Results@#A seasonal un-typed influenza virus was isolated from the infant with influenza like illness. With fluorescent PCR method , it was identified as H9N2 subtype of avian influenza virus and the case was confirmed as a human infected with an avian influenza A(H9N2) virus. Epidemiological studies revealed that the case had no clear history of poultry contact and exposure. Blast analysis shows that eight segments of the viral genome are avian origin, and 97.5%-99.8% homology with that of viruses isolated from the live-poultry markets. The virus belongs to G57 genotype, deduced amino acid sequence analysis shows that the virus has typical low pathogenic avian influenza characteristics.@*Conclusions@#Although the case does not have a clear history of contact or exposure to poultry, molecular traceability suggests that possible sources of infection may be still from poultry.

ABSTRACT

Objective This study was to understand the structure characteristics of prophages in the genome of Enterococcus hirae R17,and also to analyze their interaction relationships with the host bacterium. Methods The gene distribution and gene encoding characteristics of prophages in the genome of Enterococcus hirae R17 were identified using the PHAST software. The virulence gene, antimicrobial resistance genes, and environmental resistance genes in the prophages were also analyzed. Results Three prophages were found on the chromosome of Enterococcus hirae, including two incomplete prophage elements (Prophage-1 and Prophage-2) and one complete prophage (Prophage-3).Some function genes of bacteria were found in the sequence of three prophages, including nucleotide transportation and metabolism related genes. One incomplete prophage carrying erythromycin-and bacitracin-resistance genes was identified in the plasmid, which suggested that prophage induced gene horizontal transfer caused erythromycin-and bacitracin-resistance of Enterococcus hirae R17.Conclusion This study laid a solid foundation for the diversity analysis of prophages of Enterococcus hirae.Prophages played an important role in promotion of antimicrobial resistance of enterococci.Scientists should pay more attention to the spread of antimicrobial resistance and pathogenicity induced by prophages.

ABSTRACT

Objective The toxicological compatibility of Aconiti Carmichaeli Radix, as a toxic Chinese medicinal herb, combined with the other ingredients in Sini Decoction was investigated to elucidate the rationality of the combination of the ingredients in Sini Decoction from toxicological point of view. Methods Three kinds of experiments, acute toxicity in mice, heart toxicity in rats and aconitines level in water extract of Sini Decoction and its ingredients including Aconiti Carmichaeli Radix alone and its combination with licorice or dried ginger were adopted in this study. In the toxicological experiments, LD50 values for the acute toxicity test and TD50 values for the heart toxicity (arrhythemia as a parameter) of Sini Decoction, Aconiti Carmichaeli Radix alone and its combination with licorice or dried ginger were comparatively determined. And levels of individual aconitines of the water extracts from Sini Decoction, Aconiti Carmichaeli Radix alone and its combination with licorice or dried ginger were measured, respectively. Results The LD50 and TD50 of the combination of Aconiti Carmichaeli Radix and licorice in Sini Decoction were found to be higher than Aconiti Carmichaeli Radix alone or Sini Decoction, while the LD50 and TD50 of the combination of Aconiti Carmichaeli Radix and dried ginger appeared to be not different from those of Aconiti Carmichaeli Radix alone. The level of the main toxic compound of the water extracts for the combination of Aconiti Carmichaeli Radix and licorice, and Sini Decoction was lower than that of Aconiti Carmichaeli Radix alone and its combination with dried ginger. Conclusion The combination of Aconiti Carmichaeli Radix and licorice can attenuate the toxicity of Aconiti Carmichaeli Radix.

ABSTRACT

<p><b>OBJECTIVE</b>Through the paired comparison on the toxicity effect of Aconiti Lateralis Radix Praeparata of different compatibility proportion of Aconiti Lateralis Radix Praeparata and Glycyrrhizae Radix et Rhizoma, to observe the detoxication effect of Glycyrrhizae Radix et Rhizoma to Aconiti Lateralis Radix Praeparata.</p><p><b>METHOD</b>Paired comparison on the mouse acute toxicity of Aconiti Lateralis Radix Praeparata and Aconiti Lateralis Radix Praeparata with different compatibility proportion of Glycyrrhizae Radix et Rhizoma, to assay the LD50. Paired comparison on the rat heart toxicity of Aconiti Lateralis Radix Praeparata and Aconiti Lateralis Radix Praeparata with different compatibility proportion of Glycyrrhizae Radix et Rhizoma, to assay the TD50. We dilute medicated serum of Aconiti Lateralis Radix Praeparata, Aconiti Lateralis Radix Praeparata plus Glycyrrhizae Radix et Rhizoma (3:1), Aconiti Lateralis Radix Praeparata plus Glycyrrhizae Radix et Rhizoma (1: 1) into 5%, 10%, 20% solution with serum free DMEM, to survey the effect of different concentration of medicated serum to the pulsing rhythm of myocardial cell of original generation newborn rat, cell surviva rate and content of LDH in myocardial cells.</p><p><b>RESULT</b>LD50 and TD50 of Aconiti Lateralis Radix Praeparata can be increased after adding Glycyrrhizae Radix et Rhizoma Compared to the blank serum, medicated serum with Aconiti Lateralis Radix Praeparata can obviously increased the pulse rhythm of myocardial cell and the content of LDH (P < 0.05). The medicated serum with Aconiti Lateralis Radix Praeparata added different proportion of Glycyrrhizae Radix et Rhizoma can reduce the acceleration of myocardial cell's rhythm, which is induced by Aconiti Lateralis Radix Praeparata, and can reduce the content of LDH. With the increased ratio of Glycyrrhizae Radix et Rhizoma, the effect is stronger. But for the serum with different concerntration of Aconiti Lateralis Radix Praeparata or Aconiti Lateralis Radix Praeparata added Glycyrrhizae Radix et Rhizoma, there is no obvious effect to the cell survival.</p><p><b>CONCLUSION</b>Glycyrrhizae Radix et Rhizoma has the detoxication effect through increasing the ultimatetotaldosage of Aconiti Lateralis Radix Praeparata. The detoxication effect of Glycyrrhizae Radix et Rhizoma to Aconiti Lateralis Radix Praeparata is through restraining the increased rhythm of myocardial cell and protecting the myocardial cell.</p>
